At Mind Haven we strive to provide you with the best care possible. To help ensure a smooh appointment process, we have provided information on what to expect during initial evaluations. 

Once you have scheduled an initial evaluation, you will receive new patient forms via text or email to complete, sign, and return by email, fax, or patient portal before your scheduled appointment. ALL DOCUMENTS MUST BE RECEIVED at least 24HRS PRIOR TO APPOINTMENT. 

Please note providing an accurate history of trialed medications, side effects, and possible interactions is crucial for determining your tailored plan of care. Please be as accurate as possible.
